• After your physician writes an order for your blood draw, schedule with us

  • we must receive the order via fax (281.710.4025) or email (dsmith@dmphlebotomy.com)

• Call our office to schedule an appointment date and time

  • Please note, insurance has to be verified before appointment is scheduled

• We will dispatch our phlebotomist and drop specimen off at the lab

• MD receives results
